Financial
We participate with the following dental insurance companies: Aetna PPO & DMO, Ameriplan, Ameritas PPO, CAREington Co. PPO and Discount plans, Cigna PPO & DMO, Delta Dental Premier and PPO,Dental Health Alliance/ Fortis PPO, Dentaquest, Guardian PPO, Humana PPO, Metlife, Principal PPO, United Health Care, and United Concordia. We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. Prior to your appointment, we obtain an ESTIMATE of the amount that your insurance company is expected to pay. You will be asked to pay for the portion of the fee that is not covered by the insurance company on the day of treatment. We accept Visa/Mastercard, Discover and Cash as forms of payment.
Cancellation
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
